The puzzle is too easy, but it’s a good attention check (it’s not mate in 3):
1. Rh7+ Kxh7 2. Qxf7+ Kh8 3. Rh1+ Qh2 4. Rxh2#
My guess is that ..Qh2 is not apparent because a) it’s “useless” (other than to delay mate) and b) because in the displayed position, h2 is inaccessible to the black queen.
